Primary Care Networks

Primary care networks (PCNs) build on the core work of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for our communities. They are crucial to the development of integrated care systems (ICSs) and in meeting the ambitions in the NHS Long Term Plan.

Truro PCN is a partnership of organisations working together. These practices come together to help people live healthier lives.

PCNs also work along side staff, patients, their carers, families and the public to provide better healthcare.

PCNs have many benefits for patients, for example:

  • they can access multiple different types of support in their area
  • wider types of services available
